Questionnaire 1

 Here is a short questionnaire for you to fill out.

When you have finished with filling out the survey then you can email it back to the youth club.

How to fill out the questionnaire and email it back.

1. Download the questionnaire and put it in *My Documents* folder
2. Open the questionnaire with your word processor (Microsoft Word)
3. Fill out the questionnaire and save when finished
4. Open up your email browser/program and create a new email
5. Go through the process of attaching the questionnaire (See Help)
6. Send the email with the attachment to:

emyouthclub@googlemail.com

* You can put the document in any folder on your computer!
